How Do You Qualify For Home Health?

Answering the questions below can help determine your eligibility

1. Is leaving home difficult?

If your illness affects your ability to safely leave your home we can help you.

2. Do you have a physician?

If you have a physician that cares for you we can take direct orders from them to provide your healthcare needs. 

3. Do you require a skilled nurse?

You could benefit from a skilled nurse visiting you in your home to provide teaching on your diagnoses and medications or to help you perform injections or even wound care. 

4. Are you in recovery?

If you are recovering from a recent surgery or hospital stay then home health may be appropriate for you.

5. Do you have a physical limitation?

If your illness has left you feeling physically weak you may qualify for the services of a physical therapist to help you regain your strength.

How it works

  • 1. Who we help...

    Home Health is designed for people who have difficulty leaving their home due to an illness, but also need medical monitoring as ordered by a physician.

  • 2. Getting Started

    First we will need a referral to start your Home Health Services. Simply discuss options with your physician and request that they send Advantx a referral along with your medical info.

  • 3. Comprehensive Assessment

    Once we recieve orders from your doctor we will send out a Registered Nurse to perform a comprehensive assessment. The assessment will determine which services will benefit you the most. The nurse will start working with your doctor to develop a care plan specific to your needs. 

  • 4. Time Length

    Patients are certified for home health care for 60 day episodes. As the end of your episode approaches, you and your nurse will decide if you need continued services or if you have improved enough to be discharged. There is no predetermined limit on how many times you may be re-certified for care as long as you are making progress and continue to be homebound. Our goal, however, is to make you as independent as possible.

  • 5. What does it Cost?

    If you have traditional Medicare, your home health services are covered at 100%. That means there is no out of pocket expense to you. If you have non-traditional Medicare, or a private insurance plan, we will work with you to find an affordable solution to your medical needs.
